Case Management

BPHN provides individualized & family counseling support including:

Our Case Managers assess the needs of referred clients for housing and then identify services and resources that will enable the individual to remain in the community.

We provide direct service to clients, assessing strengths and levels of functioning to develop client centered and/or family focused personal plans.

Provide comprehensive, intense case management services, facilitate and practice follow up procedures with all clients.

Provide outreach and engagement to clients who have been identified as having a high need and assists clients in navigating multiple social service systems.

Accurately maintain client records and written reports as well as ADVOCATE on the behalf of Individuals, Families and Domestic Partnerships to increase access to services.

Direct clients to proper departments, provide resource and referrals, and provide ongoing case management; and demonstrate abilities in conflict resolution, crisis case management as well as provide in-home visits as necessary.
